PRIVACY POLICY

This privacy policy (this "Policy") describes how Cutis Wellness Dermatology & Dermatopathology, PLLC, and its related companies (collectively, "Cutis Wellness Dermatology & Dermatopathology, PLLC", "Company", "us", "our", and "we") collect, use, and share personal information and medical information of users of https://www.cutiswellnessderm.com website (the "Site") and any service or functionality, provided through the site and its mobile application (together, or individually, the "Service").

We provide a platform for patients who want to obtain information about Cutis Wellness Dermatology & Dermatopathology, and its services, contact the clinic to schedule and appointment and make payments on their invoice. Sharing health data has its benefits and risks, including those related to privacy.

This Privacy Policy outlines the type of information Cutis Wellness Dermatology & Dermatopathology, PLLC collects from individuals who intend to or are treated in the clinic, and how it is shared with other third parties, including, but not limited to, the online payment company affiliate (“Partners”). Cutis Wellness Dermatology & Dermatopathology, PLLC, does not share your online information with pharmaceutical companies, medical device companies, outside health-care providers, and research institutions .

We reserve the right to modify this policy at any time, and without prior notice, by posting an amended Privacy Policy and terms on the website and application. We encourage Members to review this policy periodically for any updates.

What kind of information we collect

Shared Data

Some of the information that Members provide about themselves may be shared with the Cutis Wellness Dermatology & Dermatopathology, PLLC community, Partners, and others (“Shared Data”). Examples of Shared Data that Members may submit at the Site, including through their health profile (“My profile”), may include:

Biographical information, e.g. photograph, biography, gender, age, location (city, state and country), general notes;
Condition/disease information;
Care provider details eg Doctor name and provider name;
Treatment information, e.g. treatments, dosages;
Individual and aggregated survey responses;
Information shared via free text fields, e.g. the forum, treatment evaluations, surveys, annotations, journals, feeds, adverse event reports; and
Connections to other people on the Site, e.g. invited care team member, mentors, feeds, subscriptions.
In the course of using the Service, Members may share information that could be used to reasonably identify them (“Personal Information”), including name, photograph, and email address. When a Member chooses to share Personal Information via a free text field (e.g. forum, treatment evaluations, annotations, journals, feeds and adverse event reports) and photos or images, the information shall be treated as Shared Data.

We automatically track and collect IP addresses, domain servers, type of web browsers, computers and domain servers used to access the site, referring source to the site and your interaction with the site and the browser, (Collectively “Traffic data”).

We also collect information you provide voluntarily in free-form text boxes on the Site and through responses including to surveys, and questionnaires.

Restored Data

Some of the information that Members enter into certain fields when registering to use or using the Site will not be shared (“Restricted Data”). The type of Restricted Data that Members may submit while using the Service may include:

Name, as collected as part of registration or in a Member’s Account Information;
Email address, as collected and verified as part of registration or in a Member’s Account Information;
Password, as collected as part of registration or in a Member’s Account Information;
Mailing address, as collected via email, forms
Date of birth, as collected in My Profile; and
Private messages.
When a Member enters Personal Information, including name and email address, as part of registering to use Cutis Wellness Dermatology & Dermatopathology, PLLC, that Personal Information is treated as Restricted Data.

Cookies

A cookie is a small data file that often includes an anonymous, unique token, which is sent to your browser from a website’s computers and may be stored on your computer’s hard drive. Two types of cookies are required to use the Site:

How your data is used

Members should expect that every piece of information they submit (even if it is not currently displayed), except for Restricted Data, may be shared with the Partners.

There are instances where both Shared Data and Restricted Data, including Personal Information, may be used and disclosed including, but not limited to, the following:

Cutis Wellness Dermatology & Dermatopathology, PLLC uses Shared Data, Restricted Data, and Site Usage internally, as needed, for research, for maintenance and operation of the Site, and to create the best possible tools and functionality for patients.
Cutis Wellness Dermatology & Dermatopathology, PLLC may use a Member’s data in the case of an emergency or other circumstance that we determine requires a member of the management team to directly contact the Member.
Cutis Wellness Dermatology & Dermatopathology, PLLC may share or disclose a Member’s data where required by law or to comply with legal process.

How Shared Data is Used

In addition to serving the individual needs of our Members, Cutis Wellness Dermatology & Dermatopathology, PLLC and its Partners are interested in better understanding the patient experience and improving treatment options and health outcomes for everyone. Cutis Wellness Dermatology & Dermatopathology, PLLC provides Shared Data, only in aggregate format, to Partners and other third parties. When sharing this information, Cutis Wellness Dermatology & Dermatopathology, PLLC removes Members’ Restricted Data to reduce the likelihood of re-identification prior to sharing information with Partners.

Cutis Wellness Dermatology & Dermatopathology, PLLC may also periodically ask Members to complete short surveys about their experiences (including questions about products and services). Survey responses are analyzed, combined with Members’ Shared Data and shared with Partners. Member participation in these surveys is not required, and refusal to do so will not impact a Member’s experience on the Site or application.

Cutis Wellness Dermatology & Dermatopathology, PLLC , like most Internet communities, is a forum open to its members, and Members acknowledge and accept that any information shared through free text or images might be connected to Members’ Shared Data (which may be shared with, or displayed for others). For example, if a Member puts his or her name (or other Personal Information) into a free text field, the Member should know this information may be included in what is shared with, displayed to Partners.

How Restricted Data is Used

Restricted Data is not automatically shared with, or displayed for other Members or Partners. Specific instances where Restricted Data may be used include, but are not limited to, the following:

If a Member provides a date of birth, the Member’s current age will be viewable via My Profile if applicable;
If a Member explicitly opts in to have their Restricted Data shared with a Partner, Cutis Wellness Dermatology & Dermatopathology, PLLC will share the information as instructed by the Member;
We do not sell or rent your restricted personal information to third parties. We do not share your information with third parties for purposes other than that of providing you the services as mentioned in this Privacy Policy document.

You may choose to authorize us to share your Personal Information like medical history with our service providers.

In order to customize your advertising interactions, we may share Personal Information with advertisers and other third parties only on an aggregate (i.e.,non-personally-identifiable) basis.

We may share your de-identified Personal Information with third parties to enable them to run statistical research on individual or aggregate health or medical trends.

We share Personal Information and Traffic Data with our business partners who assist us by performing core services (such as consultations, maintenance of site, hosting, billing, fulfillment, or data storage and security) related to our operation of the Site and/or by making certain interactive tools available to our users. Those business partners shall be bound to uphold the same standards of security and confidentiality that we have promised to you in this Privacy Policy, and they will only use your Contact Data and other Personal Information to carry out their specific business obligations to Cutis Wellness Dermatology & Dermatopathology, PLLC and to provide your requested medical care and services.

We may transfer information about you to another company in connection with a merger, sale or acquisition by or of Cutis Wellness Dermatology & Dermatopathology, PLLC. In this event, we will use reasonable efforts to notify you before information about you is transferred and becomes subject to a different privacy policy.

Other security issues

Cutis Wellness Dermatology & Dermatopathology, PLLC cannot guarantee the identity of any other Members with whom a Member may interaction the course of using the Site or who may have access to a Member’s Shared Data. Additionally, we cannot guarantee the authenticity of any data that Members may provide about themselves.

Finally, Members should know that Cutis Wellness Dermatology & Dermatopathology, PLLC takes commercially reasonable technical precautions to help keep Member data secure.

Please note Cutis Wellness Dermatology & Dermatopathology, PLLC has no obligation to delete any data or information following termination of your account. We would try our best to remove/mask any unintentional disclosure of personal information by you during consultations, but are not obligated to do so.

You may choose not to share any medical history with your consultation request.

Children

Our Services are not directed at children, and we do not seek to collect any information from children including those under the age of 18. If we learn that we have received any information from an individual under the age of 18, we will use that information only to respond directly to that child (or a parent or legal guardian) to inform him or her that he or she cannot use the Services and subsequently we will delete that information from our own servers.

Risks and benefits

While our goal is to help patients improve health outcomes, there are no certain benefits to using this website and mobile application. However, keeping track of personal well-being, treatments, and symptoms has been shown to be helpful in improving overall health. You hereby consent to store your information on our servers located at India.

In using the Site, Members are free to skip any non-required questions or data fields that make them feel uncomfortable. Members are also free to stop using this service at any time. If a Member chooses to deactivate his/her account, Cutis Wellness Dermatology & Dermatopathology, PLLC will not display the data in that account as of the date of deactivation. However, the Member’s data will remain in the system for auditing purposes conducted prior to the deactivation of a Member’s account will still include the Member’s data.

PHI Privacy Notice:

Purpose of this Notice Cutis Wellness Dermatology & Dermatopathology, PLLC is required by law to maintain the privacy of certain confidential health care information, known as protected health information or PHI, and to provide you with a notice of our legal duties and privacy practices with respect to your PHI. Use and Disclosure of PHI Without Your Consent.

Cutis Wellness Dermatology & Dermatopathology, PLLC is authorized to use PHI without your consent, authorization, or written permission in certain situations, including:

Emergency situations (in these situations, in accordance with the law we will attempt to get your written consent after the emergency service is provided and we would appreciate your cooperation when we do so);

To a relative, friend or individual involved in your care;

To a public health authority in certain situations (such as reporting a birth, death or disease as required by law, as part of a public health investigation, to report child or adult abuse or neglect or domestic violence, to report adverse events such as product defects, or to notify a person about exposure to a possible communicable disease as required by law;

For health oversight activities including audits or government investigations, inspections, disciplinary proceedings, and other administrative or judicial actions undertaken by the government (or their contractors) by law to oversee the health care system;

Your Rights

You have a number of rights with respect to the protection of your PHI, including:

The right to access copy or inspect your PHI. This means you may come to our offices and inspect and copy most of the medical information about you that we maintain. We will normally provide you with access to this information within 30 days of your request. We may also charge you a reasonable fee for you to copy any medical information that you have the right to access. In limited circumstances, we may deny you access to your medical information, and certain types of denials may be appealed. We have available forms to request PHI and will provide a written response if we deny you access and let you know your appeal rights. If you wish to inspect and copy your medical information, you should contact the privacy officer listed at the end of this Notice.

The right to amend your PHI.. You have the right to ask us to amend written medical information that we may have about you. We will generally amend your information within 60 days of your request and will notify you when we have amended the information. We are permitted by law to deny your request to amend your medical information only in certain circumstances, like when we believe the information you have asked us to amend is correct. You can appeal our denial of your request to amend the information. If you wish to amend the medical information that we have about you, you should contact the privacy officer listed at the end of this Notice.

The right to request our use and disclosures of your PHI. You may request an accounting from us of certain disclosures of your medical information that we have made in the last six years prior to the date of your request.
